fix(GenesisKit, GenesisHub, widgets): the 88 UX gaps from the 2026-10-24 audit (rail, panels, recording, permissions, hub, settings) - #563
Stacked on #562: this PR edits RecordingTargetPicker, WidgetModel+Staging, WidgetStagedViews, PermissionSettingsGuide and the hover list, which #562 adds to master.
Fixes the UX gaps found by the 2026-10-24 UX audit: 88 entries across the widget rail (R1-R11), widget panels (P1-P25), screen recording (C1-C9), permissions and approvals (Q1-Q12), the hub and settings (H1-H21) and elsewhere (O1-O10). A full report with annotated before/after screenshots per entry follows in the vault; review-relevant highlights:
New shared pieces: GenAnnounce (VoiceOver announcement + a11y.announce kit log line), ApprovalAttention (sound + reminder notification for a waiting approval), NativeSettingsSearch (search across every settings page), HubNotice (a hub note carries its tone instead of guessing it from words).
New settings (opinionated behaviour ships as a setting): widget noteSeconds (6), alertPopupSeconds (7), openShortcutEnabled + openShortcut (on, Ctrl-Opt-Cmd-W); approvals approvals.sound (Purr), approvals.notifyAfterSeconds (15).
Behaviour changes worth a look: the route approval card no longer allows on Return (click only); Open Hub in the widget moved from Cmd-H to Cmd-O; the rail badge counts only items waiting for an answer (27 instead of 99+); Dismiss / Mark complete / Remove / Discard get an Undo window.
Verified: GenesisTools tests 550/550; GenesisKit full suite run, every failure fixed and re-run green; installed and checked live on the running app (about 35 entries, list in the report). A judge pass found 0 refuted claims; its 8 caveats and 13 rule/regression findings are fixed in a follow-up commit on this branch.

ℹ️ Claims vs diff — 8 claims could not be checked from the diff (c1, c2, c3, c4, c5, c6, c7, c8).
Arrow-key routing remains undecidable from the supplied evidence.
The picker focuses the filter and places the arrow handlers on the root. SwiftUI documents focus-dependent key handling and propagation up the view hierarchy, but the available evidence does not establish whether the focused TextField consumes Up and Down before the root handler runs. A hosted-picker check is required before raising this as a defect.
When the leave prompt is open, WidgetCoordinator.handleKey handles .collapse by calling model.resolveStagedLeave("keep"). The comment is accurate, so no .keyboardShortcut(.cancelAction) change is needed.
The producer already keeps these signals consistent.
sendStagedDecisions never returns a top-level queued field. A queued delivery is stored in sessions[].result with delivered: false, which the Swift parser places in waiting. A session error is added only in the catch block, which also increments failed.
The reported false-delivery path is not reachable through this producer.

⚠️Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Check that the Return-key default needs a setting.
This change makes Return choose "Keep staged". It was "Send now". A reasonable user can want Return to send. The src/macos/**/*.swift rule says: "when a UI change picks one way where a reasonable user could want another, the choice is a setting with that way as its default."
The safe default is a defensible choice. The rule still asks for a preference that keeps the old behavior reachable. For example, add an enum such as stagedLeaveDefault ("keep" | "send"). Put it in src/hub/lib/widget/types.ts and in WidgetPreferences. Read it in this view.
If the team treats this as a safety fix and not as an opinionated choice, state that in the PR.
As per path instructions: "when a UI change picks one way where a reasonable user could want another, the choice is a setting with that way as its default."

reject stored shortcuts lacking a Command, Option, or Control modifier and
return the default shortcut instead; keep invalid values from causing the
preferences object to be rejected. The openShortcut schema is the related
preference definition, but perform validation where the native side reads the
value.

current scroll positioning and animation behavior.

- Line 226: Track item IDs while their CLI removals are in flight in
WidgetShelfStore, adding each ID in commitRemoval() before starting the removal
request and clearing it after the request completes but before refresh(). Update
refresh() to exclude both the currently undoable removed item and all in-flight
removal IDs from the displayed list.
- Line 122: Replace the hard-coded undo delay in WidgetShelfStore’s public
initializer with a dedicated widget preference whose default is 10 seconds, and
use that preference for WidgetVoiceNotesStore’s discardUndoDelay as well. Keep
the test-only delay injection available so tests can still override the

The quit flush does not deadlock on the main thread.
ToolsBridge.run is not main-actor isolated. Its process path uses private queues for pipe reads, and ToolsProcessExit.finish() resumes its continuation directly. Its server path is also nonisolated and uses the server client's event-driven queue. The reported main-thread deadlock is not supported.

Arrow navigation while the filter is focused remains undecided.
The filter TextField is focused on appearance, while the arrow handlers are attached to its parent. Apple documents that onKeyPress responds when its view has focus and that ignored events can continue up the view hierarchy, but it does not establish whether this TextField consumes Up and Down first. No code change is supported without that behavior being known.
The hub already counts every per-session send error.
sendStagedDecisions and resendAnsweredDecisions add rows.length or free.length to failed in the same catch branches that create session error entries. The reported error with failed == 0 state cannot occur through these functions.
Review fixes 2026-10-25 14:24 from Claude Opus 5.5
Round 3. eve reviewed with 2 threads and CR with 5; CR's review body had 7 additional comments, all LGTM or notes without a change (the recording picker's arrow keys are left as is, and CR itself found the send-error count already correct). This PR was also rebased onto the new head of its parent PR, which took the parent's round-3 fixes; the file set of this PR did not change. All 7 threads are valid; 6 are fixed and 1 is partly fixed.
A row title repeated in several cards gave one search result (eve)
Context: The search key was kind plus title. Every background task card has a Schedule row, so all of them shared one target: the results kept the first and a jump flashed the wrong row.
How was this fixed: a titled card passes its title to its content through the environment, the row puts it in its entry, and the key names the card. The index and the row's jump target use the same key. The search test now has two cards with a Schedule row each and expects two results with their own targets.
Confidence: 88%
A refused discard lost the edited transcript (eve)
Context: The delayed delete cleared the note's draft before the CLI answered. When the delete failed, the note came back without its unsaved edits.
How was this fixed: the draft is cleared only after the discard request succeeds, and a list refresh keeps the drafts of notes still being deleted. A new test refuses the discard and expects the note back with the edited text.
Confidence: 90%
The keys' highlight stayed on a row that left the list (CR)
Context: After a filter, grouping or snapshot change removed the highlighted row, focusID stayed set, so the overview still claimed the keys.
How was this fixed: the note the send put up is captured; both failure paths clear it when it is still the current note. The failed-send test now expects no note beside the error.
Reply (thread resolved, no thread reply): Fixed in 8e25784: both failure paths clear the Sending note when it is still the current one; the failed-send test checks it.
Confidence: 90%
An approval reminder was logged as posted before the add succeeded (CR)
Context: post() set postedID and logged the reminder as posted right after calling center.add, before its completion, and asked for no notification authorization.
Verdict: partially accepted (the bookkeeping is fixed; the authorization request is declined)
How was this fixed: postedID and the posted log line are set in the add's completion, only when it succeeded and the request is still waiting; a failed add logs the error and marks nothing. The reminder still asks for no authorization: macos.md makes tools notify authorize the first grant because a post must not raise a prompt, and an approval panel is the wrong moment for one. Without the grant the add fails, the log says so, and the panel itself still waits for the answer.
Confidence: 85%
An empty filtered PR list offered only Reload (CR)
Context: With a filter and Mine off, an empty result offered Reload, which repeats the same query.
How was this fixed: with a filter set and Mine off, the empty state names the filter and offers Clear filter; without a filter it keeps Reload, and with Mine on it keeps Show everyone's.
Reply (thread resolved, no thread reply): Fixed in 8e25784: a filtered empty list names the filter and offers Clear filter; an unfiltered one keeps Reload.
Confidence: 88%
An unknown check state was hidden (CR)
Context: The fallback sentence said the checks had no clear state and dropped the value, so a cancelled run looked like any other unknown one.
How was this fixed: the fallback names the state it got.
Reply (thread resolved, no thread reply): Fixed in 8e25784: the fallback sentence names the state it got.

Commits this round
76247045b fix(GenesisKit, GenesisHub, widgets): the 88 UX gaps from the 2026-10-24 audit: the rail counts what needs you, notes fade, undo and failures say what happened in one sentence, the recording picker, permission dialogs, hub panes and settings search follow the ux rules
aedef989e test(GenesisKit): the widget, recording and daemon tests read the new UX wording and types
1a5700071 test(GenesisKit): the rail badge, alert title, recorder failure and Open Hub tests follow the new counts, words and keys
d56e279f8 fix(GenesisKit, GenesisHub, widgets): review round on the UX fixes: undo windows follow the note setting, arrows move through decision options, the footer and hints are readable at 11 pt, a shelf or task list that cannot be read says so in one sentence, and pending ledger moves are saved when the app quits
5ee5642c3 fix(GenesisKit, permissions): review round on the UX fixes: a granted permission is rechecked for 10 s after the app returns, the recording picker drops its own ink style, and a test proves Return never allows a route approval
d9c1176eb fix(GenesisHub, settings): review round on the UX fixes: one hub notice with a tone replaces the timeline notice, hidden settings pages stop announcing, the bell popover resets when it closes, and the reports keep their order
2e385b7c0 fix(GenesisKit, permissions): bot review on the UX fixes: the approval reminder posts through UNUserNotificationCenter (the notification engine is not on this base), one relay serves every reminder, and a probe answered outside the window is rechecked
5db7ea552 fix(GenesisKit, GenesisHub, widgets): bot review on the UX fixes: a discarded note or shelf item stays gone, the cmux popover key clears when its row leaves the list, Esc stays with the widget, and the open shortcut falls back when it has no modifier
33341b2a5 fix(GenesisHub, settings): bot review on the UX fixes: settings search finds rows in folded sections and opens them, a failed daemon save can be retried, the send preview names each letter once, and the cadence menu lists a stored custom cadence
6b6e0647d fix(GenesisKit, GenesisHub, widgets): the staged list declares Reduce Motion once, the staging tests pass stagedOnly through, and the shelf tests read the new missing-item sentence
f6c339b65 fix(GenesisKit, permissions): the permissions window rechecks only the probes it answered; a grant from elsewhere is rechecked app-wide on activation
c7f915bd0 fix(GenesisKit, GenesisHub, widgets): bot review round 2: with Send at once the overview's options say Answer and Return does not send, a held dismissal cannot be answered, quitting waits for a shelf removal, the picker follows the pointer, and a stopped reminder cancels its notification
9ebeca492 fix(GenesisHub): bot review round 2: a pane notice carries its tone, the Decisions pane sends with Command-Return, and the duplicate launch notice helper is gone
8e25784b0 fix(GenesisKit, GenesisHub, widgets): bot review round 3: settings search finds a row title repeated in several cards, a refused voice note discard keeps its edits, the inbox highlight leaves with its row, a failed send drops its Sending note, an approval reminder counts as posted only once added, and the PR list offers Clear filter and names an unknown check state

…issed during its Undo is never sent, a loading settings page stays unindexed, queued sends keep their reasons behind Details, the Inbox keys follow the quiet fold, nested cards jump to themselves, and an approval notification shows no command
Review fixes 2026-10-26 02:14 from Claude Opus 5.5 judged by Claude Opus 5.5 (fable-judge)
Round 1. 4 open threads (eve 2, CR 2) and an independent GPT-6 Astra review (2 Medium). All 6 findings fixed in e384409, with fdba488 completing the settings-index fix.
The Inbox arrow keys did not follow the quiet-session fold (eve-bot-lovinka)
Context: The keys included the quiet sessions only while one of them was selected, but the list showed them whenever its own showOlder flag was open: a fold opened with the mouse was unreachable with Down.
How was this fixed: the fold state moved into HubInboxModel.showsQuiet, which the list's disclosure binds and the key order reads, so both use one state. HubInboxTimelineTests covers the order with the fold open and closed.
Confidence: 90%
A queued send's per-session reasons had no Details (eve-bot-lovinka)
Context: Only errors had a Details disclosure; a send no receiver took showed one summary sentence, and the per-session reasons were only in the log.
How was this fixed: a widget note can carry noticeDetail, shown behind a Details disclosure like NoticePill's; a new note drops it. The queued-send test asserts the hub's own reason is the detail and that the next note clears it.
Confidence: 88%
A nested card's search result and jump marker had different keys (CR)
Context: The outer card stamped its title on the inner card's entry after the inner marker was made, so the result pointed at card:Outer/Inner while the marker was card:Inner.
How was this fixed: the card entry is built with card: searchCard, so a nested card names its parent in both the entry and the marker.
Reply (thread resolved, no thread reply): Fixed in e384409: the card entry carries its parent card, so result and marker share one key.
Confidence: 85%
An approval notification showed the command (CR)
Context: The reminder's body joined the purpose and the command line; a vault request can carry private arguments, and macOS shows the body on the lock screen.
How was this fixed: the body is the purpose only; the command stays in the approval panel.
Confidence: 90%
A decision dismissed during its Undo could still be sent on close (GPT-6 Astra review, no thread)
Context: Staging, then dismissing, then closing with stagedOnLeave set to send queued the discarded answer before the deferred dismissal; Review and send and the leave question listed it too.
How was this fixed: a held decision's id goes into heldDecisionIDs (removed on Undo, on a refused change and after its commit); stagedItems, which every send, the leave question and Review read, leaves it out. WidgetStagingTests.testAHeldDecisionLeavesEveryStagedSend proves nothing is sent and Undo brings it back.
Confidence: 88%
A settings page cleared while loading was cached as indexed (GPT-6 Astra review, no thread)
Context: markSeen recorded a page as known on appear, before its asynchronous load had drawn any rows; a search cleared meanwhile left the Daemon or PR activity rows unsearchable.
How was this fixed: markSeen is gone, and every search now draws the hidden pages again, so rows a page lists after its own load (the Daemon page's tasks once its status arrives) reach the index. The index keeps what each page reported, so results still show at once. The first try (a page counts as known once it reported entries) was not enough: the Daemon page reports an overview row before its status loads.

Review fixes 2026-10-26 03:30 from Claude Opus 5.5 judged by Claude Opus 5.5 (fable-judge)
Round 2. 3 open eve threads (Medium). 2 fixed in a09ed00, each with a test that fails on the old code; 1 is a misread of the PR text (no change). The lead ran WidgetShelfStoreTests and WidgetVoiceNotesStoreTests (36 tests, 0 failures).
A list read from before a removal finished put the removed item back (eve-bot-lovinka)
Context: commitRemoval released the removing fence when the CLI removal returned. A list request already in flight, which read the shelf before the removal, was then applied with neither removed nor removingIDs hiding the item, so the row came back for a moment.
How was this fixed: A finished removal advances a list generation. A read that started under an older generation is not applied; the refresh loop reads again, so only a read that started after the removal decides the list. A new WidgetShelfStoreTests case holds a read until the removal finished and records every value of items; it fails without the fix.
Confidence: 92%
A list read from before a voice-note delete finished brought the note back (eve-bot-lovinka)
Context: deleteDiscarded cleared discarded and deletingIDs without advancing generation, so a list read that started during the Undo time and answered after the delete was accepted and showed the deleted note again.
How was this fixed: A successful delete advances generation, which refresh already compares, so the older read is dropped. A refused delete keeps the old behaviour (it reads again, so the note returns). A new WidgetVoiceNotesStoreTests case holds a read past the delete; it fails without the fix.
Confidence: 93%
The recording controls' Discard has no Undo window (eve-bot-lovinka)
Context: The PR body's summary lists Dismiss, Mark complete, Remove and Discard as getting an Undo window. Its feature list names what that Discard is: "voice-note discard" (563-pr-body.md line 20). The recording controls' Discard is a different action that already asks first.
Verdict: no change needed
How was this fixed: The PR changed only this button's tooltip. The Undo promise covers the voice-note discard, which has its Undo in WidgetVoiceNotesStore.discard. The recording Discard keeps its confirmation question (RecordingTests.testEveryControlFiresItsActionAndDiscardAsksFirst), which a deferred delete would duplicate.

Review fixes 2026-10-26 03:56 from Claude Opus 5.5 judged by Claude Opus 5.5 (fable-judge)
Round 3. 1 open eve thread (Medium) on the lead's own round-2 tests; fixed in 38324f9.
The removal race test polled its rendezvous without a bound (eve-bot-lovinka)
Context: The round-2 race tests held a list read in a loop until the removal or delete finished, with no upper bound, so a broken store would hang the suite instead of failing.
How was this fixed: Both race tests (the shelf removal and the voice-note delete) wait at most 5 s; after that the held read answers and the test's assertions fail. The two tests still pass (36 tests, 0 failures).
…-24 audit: the rail counts what needs you, notes fade, undo and failures say what happened in one sentence, the recording picker, permission dialogs, hub panes and settings search follow the ux rules
…ndo windows follow the note setting, arrows move through decision options, the footer and hints are readable at 11 pt, a shelf or task list that cannot be read says so in one sentence, and pending ledger moves are saved when the app quits
… permission is rechecked for 10 s after the app returns, the recording picker drops its own ink style, and a test proves Return never allows a route approval
…ce with a tone replaces the timeline notice, hidden settings pages stop announcing, the bell popover resets when it closes, and the reports keep their order
…l reminder posts through UNUserNotificationCenter (the notification engine is not on this base), one relay serves every reminder, and a probe answered outside the window is rechecked
…iscarded note or shelf item stays gone, the cmux popover key clears when its row leaves the list, Esc stays with the widget, and the open shortcut falls back when it has no modifier
…h finds rows in folded sections and opens them, a failed daemon save can be retried, the send preview names each letter once, and the cadence menu lists a stored custom cadence
…t once the overview's options say Answer and Return does not send, a held dismissal cannot be answered, quitting waits for a shelf removal, the picker follows the pointer, and a stopped reminder cancels its notification
…arch finds a row title repeated in several cards, a refused voice note discard keeps its edits, the inbox highlight leaves with its row, a failed send drops its Sending note, an approval reminder counts as posted only once added, and the PR list offers Clear filter and names an unknown check state
…issed during its Undo is never sent, a loading settings page stays unindexed, queued sends keep their reasons behind Details, the Inbox keys follow the quiet fold, nested cards jump to themselves, and an approval notification shows no command

GenAnnounce(VoiceOver announcement +a11y.announcekit log line),ApprovalAttention(sound + reminder notification for a waiting approval),NativeSettingsSearch(search across every settings page),HubNotice(a hub note carries its tone instead of guessing it from words).noteSeconds(6),alertPopupSeconds(7),openShortcutEnabled+openShortcut(on, Ctrl-Opt-Cmd-W); approvalsapprovals.sound(Purr),approvals.notifyAfterSeconds(15).Verified: GenesisTools tests 550/550; GenesisKit full suite run, every failure fixed and re-run green; installed and checked live on the running app (about 35 entries, list in the report). A judge pass found 0 refuted claims; its 8 caveats and 13 rule/regression findings are fixed in a follow-up commit on this branch.
