feat(worker): export worker and workflow spans over OTLP (closes #61) - #352
connected the full trace tree from api request down to worker workflows and individual activity retry attempts over otlp http json.
temporal workflows remain strictly deterministic: all tracing calls exit the workflow sandbox exclusively through Temporal sinks with callDuringReplay false, and inbound/outbound workflow interceptors handle span id generation and traceparent header injection.
workflow inbound interceptors extract the traceparent from memo, payload headers, or args and generate a deterministic 16-hex span id using temporal's uuid4 helper. workflow start and completion spans get recorded via sinks.
outbound interceptors propagate traceparent headers to scheduleActivity, startChildWorkflowExecution (nesting child workflows like memoryMaintenanceWorkflow to memoryConsolidationWorkflow directly under the parent), and continueAsNew (linking background continuations to the preceding workflow span).
activity observability passes traceparent headers and resolves parent hierarchy across headers, the in-memory lru registry, and args, reporting space.attempt, space.outcome, and on failures space.reason_code and space.category. every attempt exports its own span so retry sequences are clearly visible with error status on intermediate failures.
also resolved an issue in @space/runtime's createOtlpSpanExporter where an early flush on an empty queue left inFlight stuck as a resolved promise and blocked subsequent flushes.
all unit tests in activity-observability and workflow-observability pass, and the end-to-end integration test runs against an in-memory collector on a time-skipping test server verifying the full connected trace hierarchy. updated docs/quality/observability.md to match.
Summary by CodeRabbit
New Features
Traces now include workflow starts, completions, failures, and activity attempts, including retry outcomes and failure details.
Trace context is carried across activities, child workflows, and workflow continuations, helping connect related operations in exported traces.
Optional OTLP export includes workflow and activity spans in the trace hierarchy.
Documentation
Observability documentation now covers workflow tracing, trace propagation, and OTLP export behavior.
Adds worker-side Temporal workflow and activity spans, propagates trace context across workflow calls, and connects span export to the OTLP exporter. Adds unit and end-to-end tests and updates the observability documentation.

Summary
Workflow trace context and spans apps/worker/src/workflow-span-registry.ts, apps/worker/src/workflow-sinks.ts, apps/worker/src/workflow-interceptors.ts, apps/worker/tests/workflow-observability.test.ts
Adds workflow trace-context selection and propagation, replay-disabled workflow sinks, and a run-ID registry for workflow span contexts. Tests cover sink behavior and outbound trace headers.
Configures worker processes with workflow sinks and interceptors. Activity spans select parents from headers, the workflow registry, or arguments, and include failure attributes. The exporter flush waits for an active drain before checking the queue.
End-to-end validation and documentation apps/worker/tests/workflow-trace-otlp-e2e.test.ts, docs/quality/observability.md
Adds a Temporal test with an in-memory OTLP collector and documents workflow, activity-attempt, retry, and trace-propagation coverage.

The new end-to-end test needs a declared dependency, and successful workflow continuations currently appear as errors in traces. Resolve those issues before merging.

Full details: Linked Issues check
Explanation
Issue #61 has relevant coding requirements. The implementation adds worker workflow and activity spans, retry attributes, replay-disabled sinks, propagation tests, an OTLP hierarchy test, and the shared runtime exporter fix. However, the supplied whole-PR change summary does not show the required docs/configuration-reference.md update. It also does not show a test that replays a workflow history and proves that replay emits no spans, or a test that enforces the attribute allowlist. The change summary only reports continueAsNew propagation; it does not establish the required span link from room-background-continuation.ts background continuations.
Resolution
Update docs/configuration-reference.md for worker OTLP coverage. Add tests that replay a workflow history without exporting duplicate spans and that reject non-allowlisted attributes. Provide or implement the required span link for background continuations, including evidence from room-background-continuation.ts.
Full details: Docstring Coverage
Explanation
Docstring coverage is 33.33% which is insufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 3 functions across 11 files. (2 skipped: 2 unsupported.)
Resolution
Write docstrings for the functions missing them to satisfy the coverage threshold.

The added uuid4() draw runs before the workflow body and shifts later deterministic random values. The current consumer passes the shifted value as an activity argument. That value becomes the continuation run, message, trace, and request identifiers.
This does not establish a current replay command mismatch. Activity history records the continuation ID, and the replay test preserves the legacy activity path. The change still couples tracing to future workflow randomness.
Use the stable run ID instead. It provides the required 16-hex span-ID shape for a UUID-formatted Temporal run ID. It has the normal 64-bit collision probability after truncation, rather than an absolute uniqueness guarantee.

Replace the `uuid4()` draw used to create `workflowSpanId` with the stable
Temporal run ID from `workflowInfo`, stripping hyphens and truncating to 16
hexadecimal characters. Remove the now-unused `uuid4` import so tracing no
longer consumes the workflow PRNG.

⚠️Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Check continuationTraceparent before the memo and argument sources.
The previous run's continueAsNew interceptor sets continuationTraceparent explicitly. It is the last parent source checked here. A continued run usually carries traceparent in the memo or in args[0]. Those branches match first. The new run then attaches to the original API span, not to the preceding workflow span. The documented continuation link does not take effect in that case.
Move this block directly after the input.headers.traceparent check.

Record ContinueAsNew as CONTINUED_AS_NEW, not FAILED.
continueAsNew() throws a ContinueAsNew error out of the workflow function. This catch block receives that error. It records the run as FAILED with errorName: "ContinueAsNew". The exporter then marks the span as an error, with space.outcome=FAILED. Every continued run shows a false error in the trace.
WorkflowSpanPayload.status already allows "CONTINUED_AS_NEW", but no code sets it. Add that branch. Add a test for the execute path. The current tests only cover the outbound interceptor.

apps/worker/tests/workflow-trace-otlp-e2e.test.ts imports @temporalio/testing, but the package is absent from package-lock.json. A clean npm ci therefore cannot install it, and the worker test command can fail when Vitest loads this test.
apps/worker/src/activity-observability.ts also imports @temporalio/common. The current root-level hoisting comes from @temporalio/worker; declare it as a direct worker dependency.
