fix(server): skip the 8-minute update backup when the target adds no migrations - #134
Every service update takes the server down while the launcher copies the whole database. On stl-wsl the disk is ext4, which has no reflinks, so cp --reflink=auto byte-copies and fsyncs the 43 GB statev2.sqlite. The 2026-10-28 update from 384101 to 384103 left the server down from 22:59:06 to 23:07:05. The new server's own startup took 18 s of that.
Most nightlies add no migrations. For those, the previous version can still open the schema, so the copy buys little. It can't protect against rows the trial writes in a format the previous version doesn't decode, and the daily Restic backup covers that case.
Change
The launcher now asks the target whether it would change the schema before it stops the server.
The target's __service-preflight prints its migration manifest (migrationManifest from persistence/Migrations.ts). Preflight never boots the server.
#handleUpdateRequest runs that preflight and reads effect_sql_migrations read-only. If every (id, name) the target ships is already recorded, the launcher skips the disk-space check and writes skipDatabaseBackup: true on the pending update.
#startTrial skips backupDatabaseOnce when that flag is set.
The launcher takes the backup as before whenever it can't tell. That covers a preflight that fails, times out (10 s, under the requesting server's 30 s reply timeout) or prints no manifest, which includes every target built before this patch. It also covers a database it can't read or a name that differs at the same id. The decision is stored in service state, so a launcher that restarts mid-trial doesn't need to decide again.
restoreDatabaseBackup, the restore marker and discardDatabaseBackup all return early when the backup directory doesn't exist. A failed trial with no backup goes straight back to the previous version and keeps the trial's writes. When a trial ran, its rollback reason ends in :no-backup, so an operator can tell when a restore from the daily backup might be needed. A preflight that hits the timeout gets SIGKILL.
The running launcher does not restart on updates. This takes effect on a machine only after t3code.service restarts onto a build with this patch.
Scope and approval
Fork-only patch for fireboltdude1357/t3code, requested by Tanner after the outage above.
Verification
pnpm exec vp test run src/serviceLauncher.test.ts src/cloud/servicePreflight.test.ts src/cloud/selfUpdate.test.ts in apps/server: 28 passed.
The new test runs a failed trial three ways against a real SQLite migration history. When the target adds no migration, a full disk does not block the update, the trial sees no backup directory, and rollback returns to 1.0.0 with the trial's write kept. When the target adds one, or lists a recorded id under another name, the trial sees a backup and rollback restores the old row. Comparing ids alone fails the renamed case. A separate test checks that a failure before any trial, such as a missing target runtime, gets no :no-backup suffix.
Before: the same test against the base revision's launcher fails the no-migrations row (expected undefined to equal 'rolled-back', because the full-disk check refused the update). After: both rows pass.
End to end from source: a database migrated by the real runMigrations, a target whose __service-preflight is the real CLI (node src/bin.ts), and the real Launcher. The current database skipped the backup (sawBackup: false). With its newest migration row deleted, the launcher took one (sawBackup: true). Both rolled back to 1.0.0 with candidate-exited:1.
pnpm run typecheck in apps/server passes. vp lint and vp fmt are clean on the changed files.
Not checked: a real SEA build on ext4. The preflight took 2.4 s from TS source on this Mac, and I did not time it from the SEA binary.
Review tier: 7/10. Skipping the backup is safe only if the manifest covers every schema change, and a wrong skip shows up only when a rollback can't restore.
Two-way door: a revert brings back the backup on every update
Blast radius: launcher-managed service updates and their rollbacks
Changes made by Claude Opus 5.5 in Claude Code, running in T3 Code.
Before every service update, the launcher copied the whole database while
no server ran. On ext4 there are no reflinks, so a 43 GB statev2.sqlite
took about 8 minutes of downtime, even for nightlies with no migrations.
The target's __service-preflight now prints its migration manifest. The
launcher compares it with effect_sql_migrations before stopping the
server. When every target migration is already applied, it skips the
disk-space check and the backup, and records skipDatabaseBackup on the
pending update. Any failure in that check takes the backup as before.
Rollback without a backup already restores nothing, since restore and its
marker key off the backup directory.

[minor] Cover migration name mismatches at the same id · test-gap
The new table never exercises the name comparison that protects divergent migration histories. The implementation currently handles this correctly, but changing it to compare ids alone would preserve both tested outcomes while incorrectly skipping backups for fork databases.
When it breaks: The database records (1, 'Init') and (2, 'ForkLocal'), while the target lists (1, 'Init') and (2, 'Next').
Evidence (level 4: ran code): serviceLauncher.test.ts:465-500 supplies only matching names and a newly added id. serviceLauncher.ts:288-294 compares both id and name; lines 748 and 615 connect that result to skipping the backup. Installed effect/src/sql/Migrator.ts:263-266 skips every id at or below the latest recorded id, regardless of name. A scratch script running copies of the actual helper against real SQLite databases showed that an id-only mutation preserves both existing table outcomes but returns false for the missing collision case, where the original returns true. The base revision had unconditional backups and no such test table. The repo review rule says, "Test behavior that can break," including persistence and migrations.
Suggested fix: Parameterize the recorded migration history in the existing table and add a same-id, different-name row. Assert that the trial sees a backup and rollback restores 'before trial'.

Confirm the row passes with the current implementation and fails if serviceLauncher.ts compares only migration ids.

[minor] A rollback without a backup leaves the trial's writes in place, and "no migrations" does not mean the old build can read them · data
When the target ships no new migration, #startTrial skips the backup. #returnToPrevious then has nothing to restore and starts the previous version on the database as the trial left it. Before it sends prepared, the trial runs legacy shell import, provider recovery and delegated-task recovery, and it also starts the effect worker. The outbox effect_type column and the projection payload columns are free-text JSON, and the old build decodes them with strict unions. So a new effect, event or item variant needs no migration, yet the old build cannot decode it. Before this change, rollback always restored the backup, so the previous version only ever ran on rows it had written itself.
When it breaks: A nightly adds a variant to OrchestrationEffectRequestV2 or to a projection payload union, with no migration. The trial writes that variant during startup recovery or from the effect worker, then exits or misses PREPARED_TIMEOUT_MS before prepared. The launcher rolls back without restoring anything. The old build's claimNext marks the row running, then fails to decode it. Every later effect on that thread is blocked until a restart reconciles the row. Reading that thread's projection fails the same way.
Evidence (level 3: traced the failing path): - apps/server/src/serviceLauncher.ts:615 skips backupDatabaseOnce when skipDatabaseBackup is set. :796 restoreDatabaseBackup returns early when there is no backup directory, as the PR describes, and :806 starts the previous version on the live database.
apps/server/src/serverRuntimeStartup.ts:534-565 runs reconcileShells, providerRuntimeRecovery.recover, recoverDelegatedTasks, startEffectWorker (EffectWorker.runDaemon) and autoBootstrap. All of that comes before launcher.prepareTrial at :638, so the trial writes durable rows before it can be rolled back.
apps/server/src/persistence/Migrations/OrchestrationV2/Foundation.ts:106: effect_type TEXT NOT NULL has no CHECK constraint, so a new effect type needs no migration. Only status is constrained (:108).
apps/server/src/orchestration-v2/EffectOutbox.ts:32-122: OrchestrationEffectRequestV2 is a closed Schema.Union of literals. :268-273 rowToEffect decodes strictly. :526-548 claimNext sets status = 'running' first and decodes afterwards, so a row that fails to decode stays running. The predicate at :325-333 then blocks later effects on the same thread.
apps/server/src/orchestration-v2/EffectWorker.ts:888-903: a failed claim is only logged and retried, so effects on that thread stall and nothing reports it.
apps/server/src/orchestration-v2/ProjectionStore.ts:1045-1090: projection payloads are decoded with strict schemas, so a new variant breaks reads in the old build.
Base fe219c4 called backupDatabaseOnce unconditionally, so every rollback restored the database the old version had written.
I traced this path in the code but did not run it. It needs two things at once, a failed trial and a new variant written before prepared, so it will be rare.
Verifiers disagreed: gpt-6.1-sol refuted it (serviceLauncher.ts:286-294 checks migration history; :615 skips the backup; :374-375 leaves the database untouched when no backup exists; :870-880 restarts the previous version. serverRuntimeStartup.ts:534-565 runs recovery before prepareTrial at :638, so pre-commit writes are possible. serviceLauncher.test.ts:465-481 explicitly expects those writes to survive rollback. The base launcher backed up), then claude-opus-5-5 confirmed it on a second, blind check.
Suggested fix: Make the trade-off visible instead of implied. At serviceLauncher.ts:614, say in the comment that a rollback without a backup keeps the trial's rows, and that the previous version may not decode new variants. Record a distinct rollback reason when nothing was restored, so the operator knows to check whether a Restic restore is needed:
Also correct the PR text "the previous version can still open the database". It can open the schema, but rows the trial wrote may not decode.

Verify this finding against the current code before changing anything. In apps/server/src/serviceLauncher.ts, `#startTrial` (around lines 612-625) skips `backupDatabaseOnce` when `pending.skipDatabaseBackup === true`. `#returnToPrevious` (around lines 786-807) then restores nothing and starts the previous version on the live database. The trial writes rows before it sends `prepared`: see apps/server/src/serverRuntimeStartup.ts:534-565, which runs recovery and starts the effect worker before `launcher.prepareTrial` at :638. The outbox `effect_type` column is free text (persistence/Migrations/OrchestrationV2/Foundation.ts:106). `OrchestrationEffectRequestV2` (orchestration-v2/EffectOutbox.ts:32) is a closed union that is decoded strictly after `claimNext` has already set the row to `running`. So a new variant added without a migration can strand a thread's effects in the previous version after a rollback with no backup. Keep the backup skip, because the user approved it. Do two things: (1) extend the comment at line 614 to say that a rollback without a backup keeps the trial's writes, which the previous version may not decode; (2) in `#returnToPrevious`, append `:no-backup` to the outcome reason when `pending.skipDatabaseBackup === true`, so the operator can tell when a Restic restore might be needed. Update the existing row-based test in apps/server/src/serviceLauncher.test.ts ('backs up only when the target ...') so it asserts the new reason for the no-migrations row. Run `vp test run src/serviceLauncher.test.ts` from apps/server.

[minor] Document conditional snapshots and rollback guarantees · docs
The documentation still promises a snapshot for every update and restoration after a missed deadline. Updates proven to add no migrations now skip the snapshot, so rollback restores the previous runtime while retaining the trial's database writes.
When it breaks: A maintainer consults this document to determine whether a failed trial restores the pre-update database.
Evidence (level 3: traced the failing path): docs/internals/server-updates.md:35 and :41-49 describe unconditional snapshot restoration. apps/server/src/serviceLauncher.ts:286-294 returns false when all target migrations are already applied; :730-748 consequently skips the space check and persists skipDatabaseBackup; :612-621 skips snapshot creation. Both timeout (:796-802) and trial exit (:816-826) reach rollback (:860-880), whose restore call returns without modifying SQLite when no backup exists (:370-375). The base revision snapshots unconditionally, so this change introduces the mismatch. AGENTS.md:166 requires: "When a documented decision or constraint changes, rewrite or remove the affected text."
Suggested fix: Update the database rollback section and deadline paragraph to explain that proven migration-free targets skip both the space check and snapshot. State that failed trials then retain the live database; targets requiring migrations or whose migration status cannot be determined retain the snapshot and restoration path.

apps/server/src/serviceLauncher.ts:730Skipping the space check lets a full disk crash the launcher and stop the active server [minor]: level 3. When the target adds no migrations, #handleUpdateRequest skips databaseBackupBlocker. Before this change, that check was the only thing that refused updates on a full disk. Nothing else catches the failure, so the writeServiceState at line 751 throws ENOSPC. The error goes through #enqueue's catch into #fatal, which terminates the active server and rejects run(). That is the outage the check existed to prevent ("full disks refuse updates without an outage").
Tier 8/10 from claude-opus-5-5: Skips the pre-update backup of the live 43 GB user database. If the skip is wrong, rollback has nothing to restore, and nobody notices until a rollback is needed.
Complexity high: The update state machine now depends on a new contract across modules: the preflight wire format plus a persisted pending flag. It also runs a timed child process and a read-only SQLite comparison while the launcher handles the update request asynchronously.

[minor] Force-kill preflight when its timeout expires · error-handling
The timeout only sends SIGTERM. If preflight handles that signal without exiting, the awaited process never settles, so backup fallback and subsequent launcher transitions—including stop—remain blocked.
When it breaks: An installed target's launcher preflight ignores SIGTERM and keeps running, for example after succeeding during the earlier installation validation.
Evidence (level 4: ran code): serviceLauncher.ts:262-272 awaits execFile with a timeout and no killSignal override; its catch at 298-299 requires that promise to reject. Lines 723-728 await it inside the serialized transitions at 538-543; stop queues behind it at 568. A scratch script importing the real Launcher on pinned Node v26.12.1 demonstrated: after timeout SIGTERM was received, preflight remained alive, backup-probe calls stayed at zero, and stop remained unresolved. Manually sending SIGKILL allowed fallback and stop to complete. The base revision did not run this launcher preflight.
Verifiers disagreed: claude-opus-5-5 refuted it (- apps/server/src/cloud/serviceLauncherClient.ts:197-199: requestUpdate is the only code that sends {type: "request-update"}. A grep of non-test files finds no other sender.

The new paragraph explains an exception but leaves lines 41–45 stating unconditionally that updates are space-checked and snapshotted. This creates contradictory rollback guidance and violates AGENTS.md: "When a documented decision or constraint changes, rewrite or remove the affected text. Do not append another account of the new behavior."
When it breaks: A maintainer reads the Database rollback section to determine whether an update without new migrations creates a recovery snapshot.
Evidence (level 3: traced the failing path): docs/internals/server-updates.md:41–45 promises the disk check and snapshot; lines 52–60 append the exception and incident timing. apps/server/src/serviceLauncher.ts:286–294 returns false when every target migration is already recorded; lines 723–748 consequently bypass the space check and persist skipDatabaseBackup:true; line 615 skips snapshot creation. At base fe219c4, #startTrial called backupDatabaseOnce unconditionally and #handleUpdateRequest always checked backup capacity, so the original documentation matched the implementation. AGENTS.md:166 explicitly requires rewriting the affected text; line 164 also prohibits appending PR summaries.
Suggested fix: Rewrite the existing Database rollback paragraphs to introduce the migration-dependent backup decision first and scope snapshot, space-check, and restore guarantees accordingly. Fold in the no-backup rollback caveat and remove the incident-specific timing.

apps/server/src/serviceLauncher.ts:730Full disk now kills the active server instead of rejecting the update when no backup is needed [minor]: level 3. [minor] Only flag retained trial writes when a trial could have run · error-handling
The suffix depends only on skipDatabaseBackup, so failures before the candidate starts receive the same warning as rollbacks that retain trial writes. This gives operators a misleading repair signal even when the database is unchanged.
When it breaks: Accept an update with no new migrations, then remove its installation sentinel before handoff; alternatively, recover that pending update before its first trial with the target runtime missing.
Evidence (level 4: ran code): serviceLauncher.ts:719-754 validates the runtime, records skipDatabaseBackup, and accepts the update before handoff. Lines 629-630 reject a subsequently missing sentinel before spawning; lines 620-623 route that rejection through #returnToPrevious. Recovery also reaches it directly at lines 605-606. Line 874 appends :no-backup in both cases. docs/internals/server-updates.md:58-60 describes the suffix as flagging potentially incompatible retained trial writes; packages/client-runtime/src/state/server.ts:129-130 exposes the reason as the error message. Ran scratch/c2/verify.mjs against the real Launcher on pinned Node v26.12.1: both candidate-start-failed:no-backup and target-runtime-missing:no-backup were persisted, with no trial execution and byte-identical database contents. The base revision did not append this suffix.
Suggested fix: Track whether a trial may already have run and gate the suffix on that information as well as skipDatabaseBackup. Leave known pre-start failures unsuffixed, while preserving the warning after recovery when an earlier trial may have written.

[minor] invalid-prepared rollback after a no-backup trial is missing the :no-backup suffix · logic
#handlePrepared rolls back with the plain reason "invalid-prepared". It doesn't pass the reason through trialRollbackReason, even though this rollback only happens after a trial child has run against the live database. When skipDatabaseBackup is true, restoreDatabaseBackup leaves the trial's writes in place and the terminal reason has no :no-backup flag. That breaks the documented rule that a trial run without a snapshot gets a rollback reason ending in :no-backup.
When it breaks: The preflight finds no new migrations, so the pending update has skipDatabaseBackup: true. The trial starts, writes to the database, then sends {type:"prepared", updateId} with an id that doesn't match pending.id, for example from a buggy target build. The launcher rolls back with reason "invalid-prepared". The trial's writes stay and nothing flags them.
Evidence (level 3: traced the failing path): - apps/server/src/serviceLauncher.ts:304-305: trialRollbackReason adds :no-backup when pending.skipDatabaseBackup === true.
apps/server/src/serviceLauncher.ts:807-811 and 832-836: prepared-timeout and candidate-exited both use trialRollbackReason.
apps/server/src/serviceLauncher.ts:781-782: the invalid-prepared branch only runs when child.role === "trial" and the update is pending, so a trial child has run. It passes the literal "invalid-prepared" with no suffix.
apps/server/src/serviceLauncher.ts:880: #returnToPrevious calls restoreDatabaseBackup, which returns early when there is no backup (per the PR), so the trial's writes stay.
In git show c1f7bb7d5a, line 874 put the suffix inside #returnToPrevious for every caller, including invalid-prepared at line 777. Commit 1d97546 moved the suffix into a helper and left this caller out.
The base fe219c4 has no skipDatabaseBackup at all, so this branch introduced the gap.
docs/internals/server-updates.md:63 says "trial ran without a snapshot, its rollback reason ends in :no-backup", which is false for this path.
How reachable it is: the real client at apps/server/src/cloud/serviceLauncherClient.ts:224 echoes pending.id from the launcher context. The check pending.targetVersion !== child.version can't fail because the launcher sets both. So only a target build that sends a wrong id reaches this branch, which is rare but possible.
Optionally, add an invalid-prepared row to the existing no-backup rollback test in serviceLauncher.test.ts.

In apps/server/src/serviceLauncher.ts, `#handlePrepared` (around line 781-782) rolls back a trial with the literal reason "invalid-prepared". The other trial rollbacks, prepared-timeout (~line 810) and candidate-exited (~line 835), wrap their reason in `trialRollbackReason(pending, ...)`, which adds `:no-backup` when `pending.skipDatabaseBackup === true`. The invalid-prepared path also runs only after a trial child has run against the live database, so with no backup its writes stay without the operator flag that docs/internals/server-updates.md:63 promises. Change the call to `this.#returnToPrevious(pending, "rolled-back", trialRollbackReason(pending, "invalid-prepared"), child)`. Then extend the existing no-backup rollback test in apps/server/src/serviceLauncher.test.ts with a case where the trial sends `prepared` with a mismatched updateId and `skipDatabaseBackup` is set, and assert that the reason is `invalid-prepared:no-backup`. If the launcher restarts after a trial and its target runtime is missing, recovery can retain trial writes without warning. This narrow case merits owner awareness.

The launcher tests supply a hand-written migration list. The existing preflight test covers only blocked output. If runServicePreflight stops emitting migrations, those launcher tests still pass, but no-migration updates take a backup. Extend the preflight test with a ready case that checks the emitted migration pairs. (raw.githubusercontent.com)
As per coding guidelines, “Backend behavior changes ship with focused tests for that behavior” and “Extend before you add.”

Add a ready-output case to the existing preflight test for runServicePreflight
and assert that its emitted migrations match the expected migration pairs. Keep
the existing blocked-output coverage intact.

The launcher tests feed in a hand-written migration list, so they would still
pass if the preflight stopped printing its migrations. Updates would then take
the full backup again without any test failing.

Fixed the CodeRabbit nitpick on servicePreflight.ts:44 in 491ac24. servicePreflight.test.ts now has a ready case. It round-trips the result through JSON, as the CLI prints it, and checks that migrations equals the manifest's id and name pairs. Deleting migrations from the ready result makes the test fail.

Problem
Every service update takes the server down while the launcher copies the whole database. On stl-wsl the disk is ext4, which has no reflinks, so
cp --reflink=autobyte-copies and fsyncs the 43 GBstatev2.sqlite.
__service-preflightprints its migration manifest (migrationManifestfrompersistence/Migrations.ts). Preflight never boots the server.#handleUpdateRequestruns that preflight and readseffect_sql_migrationsread-only. If every(id, name)the target ships is already recorded, the launcher skips the disk-space check and writesskipDatabaseBackup: trueon the pending update.#startTrialskipsbackupDatabaseOncewhen that flag is set.The launcher takes the backup as before whenever it can't tell. That covers a preflight that fails, times out (10 s, under the requesting server's 30 s reply timeout) or prints no manifest, which includes every target built before this patch. It also covers a database it can't read or a name that differs at the same id. The decision is stored in service state, so a launcher that restarts mid-trial doesn't need to decide again.
restoreDatabaseBackup, the restore marker anddiscardDatabaseBackupall return early when the backup directory doesn't exist. A failed trial with no backup goes straight back to the previous version and keeps the trial's writes. When a trial ran, its rollback reason ends in:no-backup, so an operator can tell when a restore from the daily backup might be needed. A preflight that hits the timeout gets SIGKILL.The running launcher does not restart on updates. This takes effect on a machine only after
t3code.servicerestarts onto a build with this patch.Scope and approval
Fork-only patch for fireboltdude1357/t3code, requested by Tanner after the outage above.
Verification
pnpm exec vp test run src/serviceLauncher.test.ts src/cloud/servicePreflight.test.ts src/cloud/selfUpdate.test.tsinapps/server: 28 passed.:no-backupsuffix.expected undefined to equal 'rolled-back', because the full-disk check refused the update). After: both rows pass.runMigrations, a target whose__service-preflightis the real CLI (node src/bin.ts), and the realLauncher. The current database skipped the backup (sawBackup: false). With its newest migration row deleted, the launcher took one (sawBackup: true). Both rolled back to 1.0.0 withcandidate-exited:1.pnpm run typecheckinapps/serverpasses.vp lintandvp fmtare clean on the changed files.Review tier: 7/10. Skipping the backup is safe only if the manifest covers every schema change, and a wrong skip shows up only when a rollback can't restore.
Two-way door: a revert brings back the backup on every update
