Extract Manifold.Model behaviour as the backend seam (#9) - #15

Manifold.Turn and Manifold.Gate called Manifold.Llama.Client by name for every generation. Introduce Manifold.Model, a small behaviour (completion/2, stream/3) that both now call through instead, resolved from config :manifold, :model (defaults to Manifold.Llama.Client). Manifold.Llama.Client implements the behaviour and is otherwise untouched — this is a pure refactor with no behavior change for local hosting. Swapping the configured module is now the entire integration point for a future non-local backend.
