feat: Community Health Forums (Issue #159)

Description

This PR implements a full Community Health Forums feature. It introduces a frontend interface (\CommunityForums.jsx) for browsing threads and a modal for creating new discussion posts. The backend is supported by a new API route (\server/routes/forums.js) that currently handles mock in-memory posts but is structured to seamlessly transition to MongoDB. Users can now share tips and find support directly within the app.
