docs: refresh planning map and API references

Summary

• Updated map, data-contract, mutation, planning, API, workflow, and architecture docs to match the current Workflow -> Activity -> Card model.
• Documented current PlanningAction types, Agent SDK planning auth path with CLI fallback, card finalization endpoints, card output/push routes, memory route, and docs registry metadata.
• Registered ADR 0016 in the docs index and updated the docs README ADR range.
