Add --json output to ruflo doctor

[pacphi]

ruflo doctor (and ruflo doctor --component <x>) currently emits text-only, ANSI-coloured
output (✓/⚠/✗ rows with a name and free-text detail). Tools that want to check component
health programmatically — CI, editor integrations, or a wrapper CLI like agentic-kit — have to
regex-parse that text, which is brittle across releases (row wording, ANSI codes, and column
layout can all change without notice).

Please add a --json flag that emits the same information as a stable, versioned JSON array,
e.g. [{ "id": "typesafe", "status": "pass"|"warn"|"fail", "name": "...", "detail": "..." }]. A
schema version field would let consumers detect breaking format changes safely.

Context: we currently parse the text form with a tolerant regex per row and degrade to unknown
on anything unreadable, which works but is fragile — see parseDoctor in agentic-kit's src/lib/ruflo-components/evidence.mjs (pacphi/agentic-kit#236) if a reference for the current text shape is useful.

Checked against ruflo 3.44.0: ruflo doctor --help offers --fix, --install, -c/--component, --verbose and --fix-handles, but no machine-readable output.

--json is accepted without complaint and ignored. A script that asks for JSON gets text back with exit 0, which is easy to mistake for success. The option list in commands/doctor.ts (:2485 on main) is unchanged.

Proposed approach (suggestion)

• Each check already produces a status, a name and a detail before it's rendered. Collect those into { schemaVersion: 1, checks: [{ id, status: "pass"|"warn"|"fail", name, detail, fix? }] }. Print them as JSON when --json is set, and keep the existing renderer as the default.
• Use the component key as id (typesafe, memory, ...) so -c <id> and the JSON agree.
• Until then, rejecting unknown flags would at least make --json fail loudly instead of printing text.

Impact

• Ruflo users: CI jobs, editor integrations and support scripts could check doctor without screen-scraping text whose wording and ANSI codes change between releases.
• Downstream: agentic-kit could drop its per-row regex parser (parseDoctor), which falls back to unknown whenever the text changes.
