gate that cannot fire: #3562 quorum auto-escalation was never built #6101

Found by the 07:12Z quorum-stall review (handoff/quorum-stall-0800Z.md).

Rule: #3562 "two consecutive non-passing plan quorums must escalate automatically". #3562 was closed NOT_PLANNED on 2026-09-25 (parked, over the E1 budget) and nothing implements it on origin/main. So the gate cannot fire.

Measured 10-22 20:16Z to 10-23 07:01Z, 45 gate-quorum receipts: chains with 2+ consecutive failed quorums and no escalation: PMAT-5984 G2.1 freshness (#6051 to #6054, 4 in a row), PMAT-5920 (#6067 to #6073, 3 in a row; #6077, #6080; #6085, #6092). cop-state.md has no escalation line for any of them.

Acceptance: a check that, on the second consecutive non-PASS quorum receipt for one ticket chain, writes the escalation (named receipt, owner) and is exercised by a case table that turns red when the escalation is removed. LAB or GATE class to be decided by the cop under C324; it is not a PR-path check unless made a GATE by the operator.
