FLOW-003 QM-01: measure queue-model inputs (7-day, receipted) #4513

FLOW-003 row QM-01. The spec lands as docs/specifications/FLOW-003-queue-and-release-cycle-model.md (v1.1 from infra-5a).

Produce a receipted 7-day measurement of every model input: T (merge-queue build time), C (release-PR cycle), q (per-entry defect rate), φ (flake rate per run, from the FLAKE-0 ledger), λ (PR arrival rate), f / F (fix success rate and fix time), ρ (retry cost), R (re-arm delay), and the identification rate of red release cycles.

• Use the §9.1 commands (gh run list --event merge_group, gh pr list --state all, job logs).

• Every input gets a value, n, window and command, with 0 [U] fields.

• An empty window is RED, never a pass. Filter runs by headSha == PR head, because gh pr checks mixes runs from superseded heads.

• Output: queue-inputs-v1 JSON (schema set by QM-00) under evidence/, plus a summary line in the inbox.

• Read-only toward GitHub settings: no ruleset or CI change.
