binary-debt #1: ledger + DERIVED-universe guard (BINARY_DEBT ≤ 22, LEGACY_NAMES ≤ 13 at 0.70) — a new binary must be classified #4058

Part of #4057. contracts/binary-debt-v1.yaml: one row per binary (crate, bin, class, reason, target release). scripts/check_binary_debt.sh DERIVES the universe: every [[bin]]/src/main.rs/src/bin/*.rs in every crates/*/Cargo.toml, members AND excluded, plus the root; cargo metadata alone misses two. It fails when a binary is not in the ledger, or when a counter exceeds the release's ceiling (the table in #4057).

Must-RED rows: an unledgered new binary; a counter above its ceiling; an excluded crate's binary (the universe must see aprender-viz-ttop). Also fix the root Cargo.toml comment calling crates/aprender-viz-ttop a "workspace root shell (no package)"; it is a package.
