feat(v4): add cross runtime adapters

Purpose

Fleet v4 adds Multi-agent fleet management with OpenClaw upfront, followed by Claude Code, Codex, Cursor, Windsurf, Gemini CLI, and custom agent stacks. One CLI to monitor, manage, and orchestrate your entire AI agent fleet. The goal is to let operators see agent health, track runtime status, judge reliability, route intelligently, and keep accountability across the full system without locking Fleet to one runtime.

What changed

1. Added a cross runtime adapter layer with a six function contract for health, info, version, required fields, verification mode, and description
2. Added built in adapters for OpenClaw gateways, HTTP services, Docker containers, and OS processes
3. Added fleet adapters plus fleet runtime add, test, list, and remove commands
4. Surfaced runtimes in fleet agents, fleet health, and fleet sitrep with parallel probes and status deltas
5. Added optional operator constitution rules for fleet task, fleet parallel, and fleet steer
6. Added required constitution mode so scoped dispatches fail when rules are disabled or empty
7. Added fleet policy commands for show, preview, enable, disable, require, optional, add, remove, clear, title, and scope
8. Added dispatch level tests proving policy rules are included in task, parallel, and steer payloads
9. Added docs, examples, templates, metadata, and version 4.0.0 release notes
10. Hardened non TTY runtime output so piped commands stay clean
11. Fixed OpenClaw authenticated fallback probing
12. Documented OpenClaw, Claude Code, Codex, Cursor, Windsurf, Gemini CLI, and custom agent stack support by name

Verification

1. shellcheck on bin, core, commands, adapters, and tests
2. bash tests/test_cli.sh, 163 passed, 0 failed
3. Manual mixed runtime checks for adapters, runtime list, runtime test, agents, health, and sitrep
4. Manual policy management, scoping, required mode, and preview checks for injected operator rules
5. End to end fake server tests for task, parallel, and steer policy payload injection
6. Guard tests proving required mode blocks task and steer when rules are missing
7. JSON validation for examples, templates, and metadata
8. Public boundary scan for rejected wording, generated signatures, bot trailers, unsupported support order, and missing orchestrate positioning
