Backend control requests can no longer hold a worker indefinitely. Startup has a 120-second deadline, session setup 300 seconds, and steering or stop requests 30 seconds. Silent turns use read-only pings after 10 minutes, then every 5 minutes; two missed replies use the existing one-restart recovery path. An unanswered open-time ping disables only that connection's watchdog. Responsive long-running tools have no turn deadline.
Provider recovery now requires model-response evidence. A live probe retains its claim through slow startup, silence and provider retries; deferred refusals are folded before a healthy answer clears the pause. A grant older than 10 seconds is checked before sending. Codex, Grok and Kimi retry signals pause new work immediately for the selected account while the current harness attempt continues. Documentation states the coverage, including ZCode's terminal-only signals and Grok's unverified live-quota classification.
Additional fixes:
ZCode duplicate-start failures describe missing confirmation, including starts that may predate the subscription.
Cancellation wakes ZCode's duplicate wait immediately, even without a turn ID.
A duplicate steer acknowledgement remains owed until session evidence proves inclusion.
Cancellation guards discarded-input resends, including an inbox cancel claimed while the initial acknowledgement waits.
RPC input uses one growing buffer, preventing tiny fragments from accumulating unbounded object overhead.
Version: 3.0.0-dev.26.
Validation at 6ed24d8bf577c07fd19c51013228fda0ff31109c:
Local full suite: 2,562/2,562 tests pass, with zero failures or skips. All 12 documentation tests and 150 Mod tests pass. Type checking, version consistency and plugin validation pass (existing missing-icon advisory).
CI on this head: Linux is green on Node 22 and 24, including the full suite, types, plugin validation, Mod tests and live-probe validation.
The four probe regressions failed before the fix and pass after it. The cancellation regression uses a real inbox cancel claimed while the initial acknowledgement waits.
The full silent-turn recovery chain uses real unanswered RPC pings, a backend ignoring EOF/TERM, bounded SIGKILL escalation and exactly one automatic recovery. It passes in 5.63 seconds locally, 5.46 seconds in Linux Node 24 CI, and 5.75/6.05 seconds in macOS Node 22/24 CI. A separate worker test preserves a silent 25-minute job while it answers pings.
Live Codex: one-attempt job completed, with an observed answered account/read open-time ping.
Live Grok: one-attempt job completed using its configured default, with an observed answered relay/ping open-time ping. The successful no-tool smoke used yolo in a scratch directory; local read-only sandbox preflight failed before protocol startup.
Current-head macOS Node 22 and 24 each have exactly 34 failures, all matching the known baseline, with zero new failed test names. Each reports 2,521 passed, 34 failed and 7 skipped out of 2,562 tests. The documentation assertion is fixed. macOS CI remains red because of those existing failures.
Liveness state is recorded in job JSON and relay/backendLiveness events. No renderer changes are included.
Live checks still required for ZCode and Kimi:
Use a signed-in backend home and a scratch directory. Run node plugins/relay/scripts/companion.mjs task --backend zcode --model default --effort default --mode read-only --cd <scratch-dir> 'Reply exactly RELAY_LIVE_OK. Do not use tools.'. For a non-default ZCode home, set ZCODE_DATA_BASE_DIR for that invocation. For Kimi, use start --prompt-file <brief.md> with this header to select the signed-in home explicitly, then wait <job-id> --timeout-seconds 120:
backend: kimi
model: default
effort: default
mode: read-only
kimi-home: /path/to/signed-in-home
cd: /path/to/scratch
---
Reply exactly RELAY_LIVE_OK. Do not use tools.
Confirm the task completes normally with RELAY_LIVE_OK.
Inspect its job JSON or relay/backendLiveness event: the open-time ping must have enabled: true. Record the observed method: runtime/capabilities for ZCode, relay/ping for Kimi (a JSON-RPC method-not-found reply counts as an answer).
Record the result for each account used; an unsupported or unanswered ping is a capability finding, not an assumed pass.
Add capability-tested silent-turn pings, early account pause signals, and ZCode duplicate/cancel guards. Coalesce RPC input fragments to bound retained transport memory.

This change adds request deadlines and backend liveness checks. It updates provider-pause handling to rely on provider-response evidence, improves ZCode duplicate-start cancellation, and adds tests and documentation for these behaviors.
The RPC client adds configurable request deadlines and wedged-state handling. It accumulates partial output in a growing line buffer while retaining the 64 MiB line limit. ACP session requests now have deadlines.
Backend connections add ping methods and apply request deadlines. They report selected provider-response events and provider retry signals through callbacks.
ZCode checks cancellation during duplicate-start waits, accepts duplicate acknowledgements, and reports when it cannot confirm that a message started. Tests cover cancellation and acknowledgement inclusion.
The worker completes a provider probe after receiving provider-response evidence, rechecks stale admission grants, and monitors backend responsiveness. Two unanswered watchdog pings mark a backend wedged and trigger recovery. Documentation and release metadata are updated.

Explanation
The title names three themes (backend deadlines, liveness, provider-recovery evidence) that appear across the changeset. The raw summary shows deadlines in the backend transports and connections (CONTROL_MS, SESSION_MS, TransportTimeout), a new liveness watchdog (liveness.mjs, ping methods, worker integration), and provider-recovery evidence (onProviderResponse and onProviderLimit callbacks, admission grant recheck). The title omits RPC buffering and ZCode duplicate handling, but it describes the main change and is not misleading. The parenthetical issue tags add noise but do not make the title unrelated.

Update the stale sentence about control RPC deadlines.
Line 72 says "Control RPC and turn deadlines are unchanged." This PR adds control RPC deadlines:
Startup: 120 s.
Session setup: 300 s.
Steer and cancel: 30 s.
The new "Backend liveness and provider evidence" section at Lines 136-154 documents these deadlines. The two passages now contradict each other.
Reword Line 72 so it refers only to the close timings (TERM at 50 ms, KILL at 5 s). Alternatively, replace the sentence with a pointer to the new section.

Retries drop the probe's claim
When a running probe reports a retry refusal, onProviderLimit calls recordRefusal, and applyRefusal writes probe: null while the backend keeps retrying. A later successful answer cannot clear the pause because its stored claim is gone. Once the renewed cooldown expires, another job can receive a probe grant while the original attempt is still running. Both behaviors reproduced with a synthetic backend.
Preserve the live probe's owner and generation when recording its retry refusal, and release them when that attempt ends. This recovery regression needs to be fixed before merging.
The executed source/status capture documents the relevant code and unchanged production files and remains available for configured collection; upload is not confirmed.
Keep live probe claims when refusals arrive, fold deferred refusals before probe success, and expose claimed cancellation before ZCode may resend discarded input. Add inbox and full transport-recovery regressions, and reconcile the documentation.
