Add opt-in TOTP method, encrypted secret and recovery code storage, and enrollment and verification logic.
Add MFA login challenges, account management endpoints, verified-token abilities, and route middleware.
Describe the opt-in API flow, migration setup, enrollment, recovery codes, and security behavior.

Adds opt-in TOTP 2FA for API clients, including enrollment, login challenges, recovery codes, and route protection. Includes the migration and setup/security docs.
