fix(ci): publish only changed packages

Summary

• Add excludeDependents: true to Lerna version/publish config so a change in omnipartners no longer cascades version bumps to @igloo-be-omnipartners/graphql-schema and @igloo-be-omnipartners/hooks.
• Replace the double publish flow (lerna publish || true + from-package) with a script that skips publish when no package changed, then publishes only detected changes.
• Align release commit messages with [skip ci] to avoid a redundant CI run after the release commit.

Test plan

• Local: commit touching only packages/omnipartners → lerna changed returns omnipartners only (with excludeDependents)
• Local: same commit without excludeDependents → 3 packages detected (confirms the previous behavior)
• Local: lerna version patch --no-push bumps only omnipartners
• Local: script exits cleanly when HEAD is already released
• CI: after merge, next omnipartners-only change publishes only omnipartners on npm
