fix: title the comment by what it measures, drop the image

The logo could not be aligned (GitHub proxies external images and sizes them by pixel) and evlog map named the tool rather than saying what the comment is to someone who has never heard of it. The title is now Observability score, the verdict line follows, and the footer says what scored it and what that means: Scored by evlog map v0.8.0 through evloghq/action: a static read of each entry point for the wide event, context and audit trail it should emit.
