Commit 1fe52b8 pinned the actions in claude-code-review.yml and claude.yml to full commit SHAs on dev only. claude-code-action runs only when the workflow file on the default branch (main) has the same content, so since 2026-09-28 every Claude review on a PR to dev skipped and still reported success.
