Improves dictation recovery, delivery feedback, server reliability, and setup across the major-issues audit. Production defaults to Fn/Globe; Inlay Dev defaults to Right Option. A copied result fades after two seconds.

Important files:

  • Sources/Inlay/InlayController.swift
    • Recovers interrupted recordings, tracks pending results, and reports delivery outcomes.
  • Sources/Inlay/Views/MenuHUDViews.swift and Sources/InlayCore/InlayConfiguration.swift
    • Update dictation feedback, shortcut defaults, and accessibility settings.
  • Sources/Inlay/System/LocalServerManager.swift and Sources/Inlay/System/InlayTemporaryDirectory.swift
    • Add guided local-server setup and isolate development temporary files.
  • Server/src/generation-service.ts and Server/src/http-server.ts
    • Improve processing and recovery, device authorization, retention, and history.
  • Server/api/openapi.yaml and generated bindings
    • Define API v4 and keep the client and server contract in sync.

Validation:

  • bun run check, bun run lint, bun run fmt:check, bun run generate:api --check, and swift build --force-resolved-versions passed.
  • Before merging current main, the owner verified microphone testing, text insertion, interruption clipboard delivery, and the fading confirmation in Inlay Dev using the real pinned models.

Before merge/release:

  • Verify recovery and restart paths, the clipboard-preservation setting after this merge, VoiceOver, fresh-install shortcut defaults, and guided installation. Signed/notarized distribution and a second Mac remain unverified.
  • Decide the rollout for existing archives: the default retention sweep removes audio older than 90 days on startup. API v4 also requires coordinating client and server updates.

Note

Improve dictation recovery, device auth, and first-run local server setup

This PR implements the audit plan in docs/audits/major-issues-2026-10-10/ to stop losing captured speech and make dictation outcomes visible.

  • Speech recovery: interrupted recordings are salvaged and delivered to the clipboard only. Pending generations persist across relaunch as undelivered results with copy actions. Upload chunks retry transient failures for up to 60 seconds and processing polls stop after 15 minutes (ServerClient.swift, InlayController.swift).
  • Server resilience and throughput: generations process concurrently with separate speech and proofreading stage queues; invalid archive directories are quarantined at startup instead of aborting the scan; shutdown preserves sealed unfinished work as queued; warmup retries with exponential backoff. Failed auth throttles an address after 10 attempts in 5 minutes (generation-service.ts).
  • Authentication: replaces the shared token with per-device credentials (hashed in devices.json, max 64 devices), an auto-created admin token, and Bearer auth on all routes except unauthenticated /v1/liveness. API version bumps from 2 to 4 (http-server.ts, devices.ts, openapi.yaml).
  • Audio retention and storage: new audioRetentionDays preference (default 90, range 0-3650, 0 keeps forever), daily sweeps, storage-usage reporting, and purge/delete-audio endpoints. History responses now return summaries instead of full records.
  • Client setup and UI: guided local-server installation with pinned model downloads, device enrollment, custom F1-F20 trigger keys, tap-to-toggle activation, adaptive warning colors, and attention-state HUD feedback that stays expanded until dismissed (PreferencesPage.swift, MenuHUDViews.swift).
  • Packaging: removes the Swift reference server (Package.swift drops InlayServerKit/InlayServer targets); adds signed/notarized DMG distribution, LaunchAgent install/uninstall scripts, and client/server release workflows. setup.sh bootstraps the toolchain, dependencies, and both models.
  • Risk: breaking API changes — GenerationPage items change from GenerationRecord to GenerationSummary, public InlayDomain types (DictationComposer, SpokenListFormatter, TextCorrectionPolicy, PersonalDictionary.apply) are removed, and all routes except liveness now require credentials; existing clients using the shared token will stop authenticating.

Macroscope summarized 22b0134.

The remaining function-key conflict should be fixed before merging: with Input Monitoring alone, starting dictation can also run a foreground-app command.

Fix with agent prompt
### Issue 1
Sources/Inlay/System/HotkeyMonitor.swift:395-397
Custom function keys start recording through the existing listen-only event tap, so the same key still reaches the foreground app. If that app assigns an action to `F5`, choosing it as the dictation key can reload the page while recording starts. Other assigned keys can move focus or run another command.

Reserve the selected function-key events, or restrict custom triggers to keys that cannot also run foreground actions.

For each issue above, determine whether it is valid and should be fixed. If so, fix it directly.

Summary

This PR improves interrupted-recording recovery, delivery feedback, device credentials, and local-server setup.

  • Interrupted dictations can finish and wait for the user to copy them.
  • The server resumes sealed work and runs speech and proofreading in separate queues.
  • Each enrolled Mac uses its own credential to access the version 4 server API.
  • Audio retention settings and history actions let users manage saved recordings.
  • The app can download models and install its local server for a Mac.
  • Dictation can use custom function keys, tap-to-toggle, and clearer feedback.
  • Release and setup scripts package the client and standalone server.
  • The Swift server and unused domain helpers are removed from the package.
  • Supporting docs and generated files are updated for the new server and client setup.

One previously reported blocking issue remains: custom function keys are reserved only when Accessibility access is available. With Input Monitoring alone, the same key can start dictation and run a command in the foreground app.

Native macOS UI, microphone, and signing behavior were not exercised in this Linux environment.

Remote setups behind a reverse proxy can lock every Mac out of the server when one Mac keeps sending an old or revoked credential. Distribution builds may also fail notarization. Smaller issues affect event-stream reconnects, device revocation, first-run setup paths, and outdated documentation. Resolve the throttle and signing issues before merging.

- Enrollment: after legacy enrollment closes, the admin token can no longer mint or rotate any device credential (only a device renews itself); revoked device ids can never re-enroll instead of being resurrected.
- Devices: serialize store writes in a chain so a background touch() can no longer undo a revocation on disk; emit whole-second timestamps (and normalize stored ones) so whole-second ISO 8601 client decoding accepts them.
- Audio purge: retention sweep and purge-all now take per-generation locks and re-check eligibility there (incl. current retention policy), and snapshot candidate ids so recordings created mid-purge are untouched.
- Client: an incomplete upload after key release is treated as interrupted (clipboard-only, visibly reported) instead of silently inserting a truncated prefix; a lost chunk acknowledgement no longer makes recovery cancel speech the server already holds (seal falls back to the unacknowledged frame count, persisted for replay).
- Setup: XML-escape values substituted into the LaunchAgent plist template.

P1 Setup uses different folders

InlayBuild.current.dataDirectory already includes the Inlay folder. Removing its last component makes the app use ~/Library/Application Support/Models and ~/Library/Application Support/ServerData, while the scripts use folders under ~/Library/Application Support/Inlay.

Models installed by the scripts appear missing in the app, and switching setup methods selects a different archive. Use dataDirectory without removing its last component.

P1 Old jobs cancel new attempts

Cancel a queued recording while another recording is transcribing, then call reprocess before that transcription ends. The old job still waits in the speech queue. When it reaches signal.throwIfAborted(), this catch reads the newly queued record and marks it cancelled again. Its final cleanup also removes the replacement job's controller.

The retry can succeed as a request but produce no text. Tie writes and cleanup to the current job's controller, or retire the old job before reusing the recording.

P1 Import and delete overlap

completeWisprFlowImport now runs on a different queue from delete, although enriching an existing import replaces that recording's directory. Another Mac can delete the record after the import moves the old directory aside but before it publishes the replacement.

These requests can leave history pointing at missing files or bring back a deleted record. Hold the same recording lock through the import's read, replacement, and publish.

P1 Retries still discard speech

The new 60-second upload retry can outlast AudioChunkPipe's 512 buffered chunks. If the microphone sends 512-frame callbacks at 48 kHz, original and normalized chunks fill that buffer in roughly three seconds while the upload waits.

Overflow still calls failSession(..., cancelServer: true), so a brief outage cancels all captured speech before recovery can preserve it. Stop capture and keep the acknowledged audio on backlog instead of cancelling the generation.

P1 Live results appear as recovered

reconcilePendingGenerations also examines recordings still owned by pendingDictations. A completed take waiting for an earlier delivery or a current recording has no receipt yet, so it appears as “Copy finished dictation” even though automatic delivery is still scheduled.

That button can change the clipboard during another take and record “copied” before the original delivery runs. Skip live pending jobs, including after the awaited read, and expose only results whose delivery task has ended.

P2 Model checks freeze the app

refreshStatus hashes both models synchronously on the main actor. It runs during controller construction and whenever the setup section appears, so opening the app or returning to “This Mac” reads and hashes roughly 3.5 GB while the app cannot respond.

P2 History details stay stale

The selected detail now loads only when selectedID changes. Refreshing history updates summaries but leaves generationDetail unchanged. Selecting a processing recording leaves its detail stuck after completion, and “Transcribe again” keeps showing the old failure even after the retry succeeds.

P2 Shortcuts start the microphone

isUsingCustomKey bypasses the other-modifier check, and beginCustomHold has no equivalent check for toggle modes. A normal shortcut such as Command plus the selected function key can therefore start dictation too.

Reject other held modifiers for custom triggers just as the modifier-key path does, so ordinary shortcuts do not unexpectedly turn on the microphone.

P1 Old tasks cancel retried recordings

Cancelling a queued recording and choosing “Transcribe again” before its old task finishes can cancel the retry too. The old task remains in the speech queue. Its aborted signal later reaches the catch block, which reads the newly queued record and marks it cancelled. Its cleanup also removes the new task’s controller. Wait for the old task to finish, or check that each task still owns the record before changing it.

P1 Deleted imports can return

Completing a Wispr Flow import no longer shares a lock with deleting the same history item. During enrichment, replaceDirectory moves the live folder aside and later installs its replacement. A deletion between those steps can remove the record from memory while the import puts its folder back and publishes it again. Hold the existing recording’s lock across replacement and publication so deleting history cannot race with enrichment.

P1 Switching servers saves wrong tokens

If the user changes the connection while connection.enroll is waiting, replaceToken saves the old server’s returned token under the current endpoint. This overwrites the newly entered credential and can leave the new connection unable to sign in. Capture the endpoint and check it again after the await before saving the token.

P1 Client releases cannot sign

The job never imports a Developer ID certificate and private key. INLAY_CODESIGN_IDENTITY only names the identity; it does not install it on the fresh macos-26 runner. The first signing call in build-server.sh therefore cannot find the identity, so the job stops before producing a DMG. Import the certificate into a temporary keychain before building, then remove that keychain during cleanup.

P2 Setup paths disagree

InlayBuild.current.dataDirectory already ends in Inlay. Removing its last component puts models and server data directly under ~/Library/Application Support, while the scripts use ~/Library/Application Support/Inlay. Models installed through setup.sh therefore appear missing in the app and get downloaded again. Keep the app-specific directory here so both setup paths agree.

P2 App shortcuts trigger dictation

Custom function keys also trigger dictation when used with Command, Control, or Option. beginCustomHold does not check modifiers, modifier changes are ignored for custom keys, and completePress skips that check. Setting F2 as the trigger therefore also makes Command-F2 start or stop a take. Reject unrelated modifiers for custom keys, while allowing Fn when it is needed to produce the function key.

P2 History shows old results

The selected detail loads only when selectedID changes. Polling and “Transcribe again” update the summary list, but the pane keeps rendering the cached full record whenever its ID matches. A recording selected while queued or failed therefore keeps showing its old status and text after completion. Reload the selected detail when its summary changes, including after reprocessing or a bulk audio purge.

P2 Failed history loads keep spinning

A failed detail request sets an error message but leaves generationDetail nil. The view treats that as an active load and shows “Loading dictation…” forever. Track loading and failure separately and offer a retry so a brief connection failure does not strand the selected item.

P2 Valid paths break installation

The installer inserts paths directly into sed replacement strings and XML. A valid data path containing &, such as Server & Data, becomes Server __DATA_DIR__ Data, and its token path is corrupted differently. A path containing | makes substitution fail entirely. This is a non-blocking installation concern for users with those paths. Generate the file with a plist-aware serializer so paths remain literal and XML is escaped correctly.

Artifacts

Command source for ordinary paths

  • Sets ordinary path values and executes the repository substitution block directly, providing the baseline command.

Rendered plist with ordinary paths

  • Captures substitution and plist parsing with ordinary paths, showing that the paths are preserved.

Command source for an ampersand path

  • Changes the data path to contain an ampersand and executes the same repository block, providing the reproduction command.

Rendered plist with corrupted ampersand paths

  • Captures the rendered plist and parsed arguments, showing that the ampersand becomes placeholder text instead of remaining literal.

Command source for a pipe path

  • Sets a data path containing a pipe and executes the repository block, providing the substitution-failure command.

Substitution failure with a pipe path

  • Captures sed exiting 1 with an unknown-option error and empty rendered output, confirming that the valid path prevents plist generation.

- Setup paths: app-managed server files now live under Application Support/Inlay like the scripts (both builds, one LaunchAgent); legacy installs are adopted via their marker, and uninstall clears both marker layouts.
- Server: cancelled-then-reprocessed recordings can no longer be cancelled by their draining old task, queue positions follow the replacement attempt, and Wispr Flow enrichment holds the generation lock so it cannot race deletion.
- Upload: pipe backlog during connection retries now salvages the acknowledged audio instead of cancelling the generation; live takes no longer surface as recovered results; connection saves keep pending results for the same endpoint and stale credentials cannot delete them.
- Hotkeys: custom trigger keys are swallowed so they no longer also run the focused app's command (separate active tap, Accessibility-gated), modifier chords no longer start dictation, and modifiers added mid-take no longer cancel it.
- History/setup UI: detail reloads when its summary changes, failed detail loads show a retry instead of spinning, and model integrity hashing runs off the main actor with cancellation.
- Release/install: client release imports the Developer ID certificate into a temporary keychain (new INLAY_CODESIGN_P12_* secrets), and install-service.sh escapes paths for sed/plist and rejects newlines.

P1 Setup attempts stop each other

performInstall now waits for model checks before setting .installing. The setup button stays enabled during that wait, and install() does not reject another call. Repeated clicks can start two installs that both pass the port check. One can stop the other's service, and either attempt's cleanup can remove their shared setup files.

Mark installation as running before the first await and reject overlapping calls.

P2 Cached history hides failed reloads

loadGenerationDetail now keeps the cached record during a reload. If that request fails, HistoryPage shows the cached record before checking generationDetailFailedID, so the error and “Try again” controls stay hidden.

After a recording finishes or is reprocessed, one failed request can leave its old status and text displayed. Refreshing the list does not retry once the summary stops changing. Show the reload error alongside the cached detail, or clear the cached detail on failure.

Babysit pass complete: marked ready for review and brought to green. Three review-fix commits were added on top of this PR — a895a30 (Codex review), 1d10e45 (Greptile review), bb68fac (Greptile follow-up) — each fix wave got an adversarial re-review before pushing.

Wave 1 — auth, purge, and speech-loss races (a895a30)

  • Closed-enrollment bypass: with legacy enrollment closed, the admin token could still mint/rotate a credential for any existing device id (and resurrect revoked devices), defeating the devices-only restriction on the admin actor. Now only a device's own credential renews it; revoked ids return 403 device_revoked.
  • A background touch() write could land after a revoke and restore the old token hash on disk. Device-store writes now serialize in a chain, snapshotting state when each write runs.
  • Device timestamps were emitted with milliseconds while the client decodes whole-second ISO 8601. Now whole-second, with stored fractional values normalized on the way out.
  • Retention sweep / purge-all held the global queue but no generation locks, so a purge could delete audio under a concurrent reprocess. Bulk purges now lock per generation, re-check eligibility (including current retention policy) under the lock, and snapshot candidate ids.
  • An incomplete upload after key release silently inserted a truncated dictation as success. It now behaves like a salvaged take: clipboard-only, visibly reported.
  • A lost chunk acknowledgement could make recovery cancel (and delete) speech the server already held. Sealing falls back to the unacknowledged frame count on 409 incomplete_audio, persisted for replay.
  • LaunchAgent plist substitutions are XML-escaped.

Wave 2 — Greptile findings (1d10e45, bb68fac)

  • App-managed server paths now use Application Support/Inlay/{Models,ServerData} like the scripts (both builds, one LaunchAgent). Legacy dogfood installs are adopted via their marker, and uninstall clears both marker layouts. Note: a legacy install's models under the old Application Support/Models show as missing and need a move/re-download on reinstall — no migration was added.
  • Cancel-then-reprocess can no longer be cancelled by the draining old task; queue positions follow the replacement attempt; Wispr Flow enrichment holds the generation lock so it cannot race deletion.
  • Upload pipe backlog during connection retries salvages the acknowledged audio instead of cancelling the generation. Live takes no longer surface as recovered results; connection saves keep pending results for the same endpoint, and stale credentials cannot delete them.
  • Custom trigger keys are swallowed (separate active event tap, Accessibility-gated, own run-loop thread with explicit shutdown) so they no longer also run the focused app's command. Modifier chords no longer start dictation, and modifiers arriving mid-take (including companion keyDowns) no longer cancel it.
  • History detail reloads when its summary changes; a failed reload now reaches the error/retry pane instead of silently keeping the cached record. Model integrity hashing runs off the main actor with cancellation.
  • Local-server installs are serialized: install() claims the installing state before its first await, overlapping calls are rejected, and detect() cannot reset that state mid-install.
  • Client release workflow imports the Developer ID certificate into a temporary keychain with cleanup. New secrets required: INLAY_CODESIGN_P12_BASE64, INLAY_CODESIGN_P12_PASSWORD (existing INLAY_CODESIGN_IDENTITY / INLAY_NOTARY_* still needed).
  • install-service.sh escapes paths for sed/plist and rejects newline paths.

Verified

  • bun run check, bun run lint, bun run fmt:check, bun run generate:api --check, swift build --force-resolved-versions (incl. one clean-build) pass; full CI green on bb68fac (all 10 runnable checks, incl. Greptile).
  • Enrollment/revocation exercised live against an isolated dev server: revoked re-enroll blocked, admin rotation after closure blocked, device self-renewal works, timestamps whole-second on wire and on disk.

Intentionally skipped / unchanged

  • CodeRabbit could not review (117 files exceed its 100-file limit) and Macroscope was skipped for insufficient credits. Findings came from local Codex/Claude adversarial review rounds plus Greptile instead.
  • On the rare fallback-seal path the server still explains omitted original audio with the existing "format too large" wording, which is inaccurate there. Speech is preserved; fixing the message is a small follow-up.

Still owed before merge/release (unchanged from the PR description, plus these new behaviors)

  • Runtime verification in Inlay Dev: interrupted/truncated dictation, custom F-key behavior (F5 in a browser, Cmd+F5, hold + Shift, double-tap, Fn on laptop keyboards), closed-enrollment UX, local-server install/adopt/uninstall.
  • Stricter enrollment semantics: a revoked device id is now permanent, and after closing enrollment there is no admin recovery other than resetting devices.json.
  • Provision the two new codesign secrets before tagging a client release; signed/notarized distribution and a second Mac remain unverified.

Update the recording lifecycle text to match the new server behavior.

Line 29 says "One job at a time runs Parakeet …". The server now runs jobs at the same time and serializes only each helper stage, so speech for one take overlaps proofreading for another. Line 32 says "Restarting the server marks unfinished generations failed". GenerationService.initialize now resets non-terminal records with sealed inference.wav to queued and processes them again. Line 32 also says there is "no … automatic audio-upload retry". The client now retries chunk uploads for up to 60 s and keeps pending generations across relaunch. Correct these statements so operators know which work survives a restart.

- Align first-run model downloads with both development runners, including exact-manifest legacy fallback and Linux defaults.
- Sign the distribution app bundle with a secure timestamp.
- Type revoke-device IDs as strings, preserve arbitrary accepted IDs as one URL segment, and keep accepted IDs inside router/request-line limits.
- Restore documented event heartbeats and let valid device credentials bypass admin-token guessing throttles without resetting them.
- Correct stale lifecycle, API-version, service-installation, and concurrency documentation.

Follow-up cleanup after the babysit summary:

  • Removed the 23-file audit working-docs tree under docs/audits/major-issues-2026-10-10 in 985b176. The PR now retains only product/documentation updates (AGENTS.md, READMEs, architecture/contract/development docs, and the pre-existing Wispr import plan).
  • Removing those files brought the PR under CodeRabbit's file limit. Its first actual review produced 8 findings, addressed in f43d26f: outer-app distribution timestamping; setup/dev-runner model-path alignment (including exact-manifest legacy fallback and Linux defaults); string device IDs with safe single-segment revocation and revocable size bounds; roughly-two-second event heartbeats; valid device credentials bypassing the admin-guessing throttle without resetting it; and stale lifecycle/concurrency/API-version docs.
  • Final head f43d26f is MERGEABLE/CLEAN. Local bun run check, lint, fmt:check, generate:api --check, Swift build, and shell syntax checks pass. Isolated live probes verified encoded/long device-ID revocation and MLX fallback when the canonical model directory cannot be searched.
  • No new CodeRabbit or Greptile comments were posted on f43d26f.
