sync: upstream pstack 0.15.18, ketchup shows action items as before and after visuals - #182

vendor/pstack matches ea32d9562a1c9f077d7cdd3be53d07383eb18866. Ketchup shows each action item as a before and after and allows visuals made from what the agent already has.
The change note no longer puts a Mermaid diagram, a chart, or a generated image in the otherwise case. That case stays a table of numbers, a diff, or a diagram. The prefer line names the other list.

Brings the vendored upstream skills from pstack 0.15.17 to 0.15.18.
Upstream made one change. The catch-up skill, ketchup, now shows each action item as a before and after, with a screenshot or video when the change is visible and a table, diff, or diagram otherwise. It may make those visuals from what it already has, and stays read-only apart from that.
vendor/pstackmatches upstream at ea32d9562a1c, file for file.skills/ketchup/SKILL.mdis the rebuilt copy. No override changed.Known limits. The new ketchup text names visuals in general terms and no T3 tool. A T3 wording for it is a follow-up. The badge is still edited by hand, and a build check for it is a follow-up.
