So I questioned to save to the extent

I'm really sorry but I don't understand what you are trying to do. If you want to add a new field to a table it has nothing to do with this module. Simply add the field using phpmyadmin.

Yes, let's suppose we inserted this new field in the database, how do I save in this field, by using the extension?

This extension doesn't save data in the database. You just save as you normally would in Yii.

$user = new User('create');
$user->my_field = 'some value';
$user->save();
