Clear pending when a start fails. A recoverable error blocks later starts on the same handle.
start_multi_mask_audio_windows moves running into pending before its remaining fallible calls. Those calls can return Err and leave pending set. These calls are the two embed_multi_mask_audio_batch calls, check_multi_mask_counts, and the session.run launch. The guard at the top of the function then returns MissingOutput for every later start on that handle. A launch error can come from Batches::batch, which selects and creates the spare batch.
Reset pending on every Err return after the move. One option is to move the code after the move into a helper and clear pending when that helper fails.
The runner has the same exposure. If run_multi_mask_with returns early from hard_decode or start_batch while a batch runs, that batch stays in running. The next file's first start moves it into pending. If the same handle serves that file, its second start returns MissingOutput. Clear running and pending on that early-return path too.

The 5090 had no measured recipe, so it ran the TF32 early trunk default. A tune on the card timed FP16 wide tiles 1.25-2.3x faster on the same-channel 32-channel layers at every batch and 1.07-1.4x faster on the 64-channel layers from batch 4. The 128- and 256-channel layers stay on TF32 Winograd, which wins at batch 32. The recipe matches only cc 12.0 with 170 SMs and the RTX 5090 name. On all 216 voxconverse-dev files, Cuda drops from 39.0 s to 35.0 s with DER unchanged at 7.0123 and 3 RTTMs changed; CudaFast drops from 20.4 s to 18.3 s with identical RTTMs.
The multi-mask loop waited for each embedding batch, then staged and launched the next, so the embedding stream sat idle for about 0.6 ms per full batch while the host copied the output and staged the next span. Now the next batch starts first: its launch queues the running batch's output copy into pinned memory, and the wait syncs only that copy. Full batches alternate between two 32-chunk batches, so the earlier batch keeps its own inputs for the FP16 range recompute, which a new GPU test drives after the next batch has overwritten the shared activations. On an RTX 5090 with all 216 voxconverse-dev files, interleaved runs were faster in every pair: Cuda by 0.8% and CudaFast by 1.3%, with identical RTTMs.
A batch stream started each file's segmentation only after the previous file's embedding finished, so every file began with the GPU mostly idle while its first segmentation batch ran. In CUDA concurrent mode, one worker now segments the files in order and takes the next file while the current file's last embedding batches run. The bounded window queue limits how far ahead it gets, so results and their order don't change. On an RTX 5090 over voxconverse-dev, interleaved pairs went from 34.6 to 34.2 s for Cuda (4/4 pairs faster) and from 18.0 to 17.5 s for CudaFast (3/3 pairs), with identical RTTMs.
On an RTX 5090, m16n8k16 FP16 products with FP32 accumulation issue at the same rate as m16n8k8 ones, so they do twice the work per tensor instruction. The h16 trunk kernels issued nine m16n8k8 products per staged chunk and were tensor-pipe bound at batch 32. A new sm120 wideconv variant holds each even tap's fragments and issues them with the next tap's as one m16n8k16 product: four k16 and one k8 product per chunk. The sm120 build compiles every wideconv kernel differently, so only the RTX 5090 binds that variant, by its exact cubin. Other devices, other cc 12.x GPUs included, keep the sm80 variant, whose PTX and cubins are unchanged, and sm120 builds still embed it for them. On voxconverse-dev the Cuda run went from 34.0 to 30.9 s and CudaFast from 17.6 to 16.1 s. DER is unchanged and one of 216 RTTMs differs, by 0.003 points.
With the sm120 FP16 kernels doing twice the work per tensor instruction, a tune times FP16 tiles ahead of TF32 Winograd on the later trunk layers too: 1.15-1.35x at batch 32 for 128 channels and 1.1-1.15x for 256, and 1.07-1.5x on the stride-2 layers. The RTX 5090 recipe now runs the 32-channel layers, stride 2 included, on FP16 at every batch, the 64-channel ones from batch 4, the 128-channel ones from batch 8 and the 256-channel ones from batch 16. Below those batches TF32 stays within about 10% or wins. On voxconverse-dev the Cuda run went from 31.0 to 27.3 s and CudaFast from 16.0 to 14.2 s. Cuda DER went from 7.0123 to 7.0112 with 5 of 216 RTTMs changed, the worst file by +0.038 points; CudaFast DER is unchanged.
The 128- and 256-channel same-channel layers were the largest share of the 5090 embedding trunk after the direct FP16 kernels reached the tensor pipe's limit. Fused Winograd F(2x2, 3x3) on m16n8k16 FP16 products does 2.25x fewer multiplies with the same operand rounding as the shipped one-product TF32 Winograd, and accepts the direct kernels' half tensors and range word, so it slots in per layer. The kernels exist only in the sm120 wideconv variant, which only the 5090 binding loads; xtask lets that bound top tier export entries the lower tiers lack, so the sm75 and sm80 artifacts are unchanged and the 88 existing sm120 kernels keep identical SASS. Weights whose transform could leave the FP16 range, devices without the sm120 variant and tune files under that limit keep the direct FP16 tiles. On voxconverse-dev the 5090 Cuda run went from 27.0 to 25.7 s (4 of 4 pairs) with DER 7.0112 to 7.0123 and 7 of 216 RTTMs changed, the worst file by +0.14 points; CudaFast went from 14.0 to 13.2 s with DER unchanged and one RTTM changed.
Add 64-channel instantiations of the RTX 5090 FP16 Winograd kernels and select them for the same-channel layer2 convolutions, which spend more GPU time than the 128- and 256-channel layers combined. The residual epilogue now prefetches the residual into L2 during the last input-channel chunk and stages a half residual through shared memory in coalesced 16-byte pixels. At batch 32 the 64-channel layers run 1.05-1.2x faster than the direct FP16 tiles and the 256-channel residual layer 1.09x faster. On voxconverse-dev Cuda goes from 25.3 to 24.9 s with DER 7.0123 to 7.0119 and 4 of 216 RTTMs changed, and CudaFast from 13.1 to 12.8 s with DER unchanged. Only the sm120 artifacts change; the other kernels' SASS is identical. The FP16 Winograd accuracy test compares the 64-channel shape against the FP32 Winograd kernel, since that shape has no spatial tiles.
List the full-suite CUDA and CUDA Fast results on an RTX 5090 next to the RTX 4090 rows, with the 5090 hardware and run counts. The raw result files get matching blocks. The 5090 rows include pipeline and kernel changes the 4090 rows predate, which the page says.
Rerun the full suite on RTX 4090 containers with the same 16 cores and 8 GiB at this branch's code, so the 4090 and 5090 rows share one speakrs version. DER and its parts are unchanged on every dataset. On VoxConverse dev, run interleaved with master in one container, cuda went from 39.4 to 38.2 s and cuda-fast from 21.0 to 20.0 s with identical RTTMs. The single runs on other datasets landed on different hosts and drivers and range from 6% faster to 6% slower than the earlier rows.
Only the sm75/sm80 stride-2 pin test uses Fp16Policy, so the module-level import was unused under cuda-sm120 alone and failed clippy with -D warnings.
cargo rdme generates README.md from src/lib.rs, so the refreshed RTX 4090 numbers and the RTX 5090 profile mention must live there for the readme check to pass.

A multi-mask run that returns early, from a failed start or from the runner, can leave running or pending batches on the embedding model. The next recording's second start then fails with MissingOutput, so one failed file breaks later files on the same pipeline. Drop that bookkeeping when run_multi_mask fails; the batch buffers stay with the handle.
