Remove print, sign, scan in favor of pdf form filler

Thanks for the review. I agree with most comments, but... **MacOSX or Linux users:** **MacOSX and Linux users:** I think either of these is correct. "Users of Mac or Linux" use either Mac or Linux. I don't think we need to restrict this section to Users of both Mac and Linux. Whatev... 4. Keep the completed form for your records. The user should keep the completed form, not a copy of the form. What is sent to Secretary is a copy of the form. The user keeps the original completed form.
Updates based on feedback from @sebbASF which should resolve all issues
Clarify that the signature must not be typed.
