replace naive swbreakpoint impl by instruction replacement on gdbrsp

Gdbrsp module checks on the breakpoint list whenever the value of PC changes during execution, resulting in poor simulation performance.

Performance can be enhanced by replacing opcodes by ECALL or EBRAK (riscv only) instructions in each of the breakpoints' locations.
