Listing sessions via the scan-and-repair path and then filtering by cwd in the client can take a long time. By comparison using a cwd-filtered DB query is much faster (e.g. 40ms vs ~30s on my machine with a few hundred sessions).

Instead of listing all available sessions from their transcripts and then filtering by
cwdin the client, Codex can return the filtered session list from a database query, which is much faster for users with large numbers of sessions (e.g. ~30s down to ~40ms on my machine with a few hundred sessions).This PR also fixes the issue addressed by #329 (cwd filtering works correctly during pagination).
