Add additional fields to milking-visits

Herd management systems today get these data points from parlor exchanges and the data is used in parlor performance reports

• manualDetach - a user has manually detatched the unit
• reattach - the unit detached and the user had to manually reattach it
• manualKey - the animal was not identified using RFID (or equivalent technology) so had to be manually entered
• detachTime - when the unit was detached from the animal

Other possibilities:

• Use boolean values rather than 0/1.
• Consider an array or similar of error results / codes
• Consider manualIdentification or manuallyIdentified instead of manualKey

We would like to add the detachTime but the other values could become enumerated remarks in the icarMilkingRemarksType enumeration which is already supported by an array of remarks (warnings / errors / exceptions).
