Overview
Regenerates the Java SDK to 17.13.1 (Fern CLI/generator bump) and wires new API surface from the updated OpenAPI spec.

New capabilities: client.lang2FhirBatch() for async batch Lang2FHIR jobs (create, upload items, finalize/cancel, paginated status/results). Implementation guide version publish/get and custom profile version CRUD under profiles().versions(). Lang2FHIR multi/document flows gain optional primaryPatient; patientReference is a deprecated alias and must not be combined with it. Document extraction types/docs now include RTF and XML/C-CDA (instance-specific).

FHIR→OMOP: CreateOmopRequest adds optional vocabVersion; request/response models and reference docs expand for broader resource coverage, richer tables/mappings/diagnostics, and stricter eligibility semantics.

Client/runtime: PhenomlClient / AsyncPhenomlClient and ClientOptions implement close() for owned OkHttp resources. Credential auth drops configurable grantType (always client_credentials). Custom instanceUrl only replaces the default URL template when environment is unset or DEFAULT.

Minor/breaking-adjacent: Crosswalk error handling maps some former gateway/not-implemented statuses to InternalServerError. SDK identity headers (User-Agent, X-Fern-SDK-Name) updated to the Maven artifact coordinates.

Stream uploads omit required multipart fields

The stream-based uploadItem overloads send only a file part and ignore document, create, request_id, and id. They also take an unused file argument. Document items require a document JSON part, create items must not send a file, and retries need request_id, so these overloads cannot satisfy the API contract and will be rejected.
