[Feat] 약 상세 정보 조회 기능 제거 및 병원별 처방 그룹 조회 기능으로 전환

약물노트 병원 카드 클릭 시, 같은 병원 + 같은 처방일 기준으로 묶인 약 리스트와 각 약 등록 시 작성된 메모를 보여주는 그룹 상세 조회 기능

기존 약 상세 정보 조회 기능 제거 (MedicationQueryService, MedicationDetailResponse, GET /api/medications/{id}/detail)
GET /api/medications/group 엔드포인트 설계 (hospitalName, prescriptionStartDate 파라미터)
그룹 조회 서비스 로직 구현 (동일 병원 + 동일 처방일 기준 그룹핑)
응답 DTO 구성 (약 리스트 + 각 약의 memo 필드 포함)
Swagger 문서 반영

📒 기타
병원 정보 없는 약(hospital_name = NULL)은 "병원 정보 없음" 그룹으로 별도 처리
