feat: run any action from a fuzzy command palette - #37

ctrl+p (configurable as paletteKey) opens a searchable list of every action the current screen binds, plus "go to" entries for work items, pull requests and builds. Entries come from the same bindings as the "?" panel and run by replaying their key, so no view changed and the two cannot drift. Recently run entries are listed first and persist beside the config.
