feat(gate1): attach injectable RepoForge/Engram producer output - #12

B/C/D prompts include producer_output. CONTROL has none. Live CLI runs argv subprocesses; tests inject or mock. Unavailable producers are typed, not silent. Not a utility verdict and not a WAVE3 rescore.
