#9 rebase 的「双方内容并列」策略在 ops/README 索引表留下副作用——embedding 出现两行（各写 2 平台）、「硬件平台合计」注记双条。本 PR 合并为单行三平台终态（含 PLATFORM/MERGE 链接）；sdpa_math 双平台已有 PLATFORM.md，按 embedding 范式补齐 MERGE.md 七步，使三个多平台算子文档形态统一。
