USER STORY: enter payment info

As a site user, I can easily enter payment information so that I can check out quickly and smoothly.

Acceptance criteria:

1. The payment form can be read and filled in relatively quickly (especially if the user's delivery info is saved via profile).
2. The payment form does not have confusing or distracting elements.
