Improve client navigation accessibility
Summary
Add a skip-to-main-content link and focus the main content after pathname navigation.
Announce route changes through a polite live region and update document titles for routes, vaults, and validations.
Restore scroll position on browser back/forward navigation and handle hash links.
Add focused tests for navigation accessibility, scroll restoration, and route titles.
Verification
Passed: 8 focused tests across the layout, scroll restoration, and route-title suites.
