The original author is back again

Hello @versat , last year I found that the original author, David Kubicek, is back again and opened a repo on Github at https://github.com/DavidKubicek/cntlm . I contacted him and he is positive on joining forces to evolve the project together.

I have seen it on SourceForge. Good to know that there is a GitHub repo now.
Not sure how to bring the changes together. Maybe (some) PRs can be re-created against his repo. Or specific commits could be cherry-picked.
