Fast Secure Contact Form Newsletter Add-on no longer working

I have been using the Fast Secure Contact Form Newsletter Add-on for over a year with no issues. Recently there was a series of updates to Fast Secure Contact Form in February and now whenever a user enters data in the form, I receive an e-mail notification rather than the Constant Contact list getting updated automatically.

I have verified my CC credentials and all of that checks out.

What can I do to resolve this issue?
