[luv-31] Default prompt-less sessions to max effort + random color (not ultracode) (#30)

* [luv-31] Drop the /effort ultracode startup input; keep max as the default

Prompt-less interactive sessions (plan + bypass) auto-ran "/effort ultracode"
as their first startup input, entering ultracode mode. Remove that so those
sessions just stay at the --effort max CLI baseline already passed on every
launch path. initial_args is now empty when there is no prompt, so no startup
command is consumed and the session opens at max effort.

Update the README "How it works" line to match.

* [luv-31] Restore the random /color startup input and bump to 0.0.22

Bring back the COLORS list, pick_color() helper, and the random import that
#28 removed, and wire "/color {pick_color()}" back into the prompt-less plan
and bypass launch paths so each session is visually distinct again — while
keeping --effort max (not ultracode) as the baseline.

Bump version 0.0.21 -> 0.0.22.
